Rule identified 3 matching files, only worked on 2

I recently upgraded to Hazel 5.0.3 from Hazel 4 and have been experiencing processing delays, but this is very strange. I downloaded 3 PDFs from the same website at the same time (my husband's 3 most recent pay stubs). I have a rule to rename and sort them into folders by date. Hazel saw all 3 files, said they all matched the rule, and moved 2 of them. The other is still sitting in my downloads folder and Hazel still says it matches, but nothing happens, even when I manually tell it to run the rules.

I am not sure how to proceed, since it appears that Hazel is working, but it really isn't.

Update: It appears this problem is more widespread. Out of several bank statements, after starting and stopping, Hazel picked up on and moved 3 of them. It didn't touch the others.
