Simple file rename rule not working. Built from scratch.

This rule worked great under v4. I imported it into 5.01 and it is not working. I then built if from scratch, still not working. Any ideas?

If
[All]
[Extension][is][mp4]
[Date Last Matched][is blank]

Do:
[Rename] with pattern [#] [name][extention]

nothing to show in the log as it ignores the rule unless I force it. Then it works as expected, but goes silent again.

Thanks but nothing I could do would resolve this issue. Downgraded and everything is back to normal. I know you are working hard so I am not going to add to your workload as there is not much difference for my uses between 4 and 5. I will upgrade when everything calms down. Thanks for all of your hard work.
