Hazel Rule Not Triggering on Specific Folder

I've created a Hazel rule to automatically tag all downloaded PDFs with a specific tag ("Downloaded PDF"). I set the rule to monitor my Downloads folder. However, I noticed that some downloaded PDFs are not getting tagged. Any idea?
