Adding Color Label has Broken

I used a rule that changed the color label from nonexistent to blue for two weeks and had no problem, but it has since broken. Somehow, it runs the Applescript I have, yet doesn't proceed to change the color label as specified in the next action.

I've had the same problem on both Yosemite and El Capitan, and have uninstalled/installed Hazel. I simply do not understand why it broke. If anyone has any suggestions, they would be greatly appreciated.

I suggest checking Hazel's logs to see what happened. Email/post anything you find there related to the file in question.
