Every time i reboot my computer, hazel applies rules to files in folders. I think i have my rules in place to rename/move when files are added to the folders.

I now have names like 2015-07-29 iPhone Claudine 2015-07-29 iPhone Claudine 2015-07-29 iPhone Claudine 2015-07-29 iPhone Claudine 2015-07-29 iPhone Claudine IMG_8302.mp4.

What do I do wrong?

Your rule still matches the renamed file. You should add conditions to the rule to filter out files you already renamed. You can either match the pattern of the newly renamed files or add a tag to identify them.
