Import to Photo app

I'm trying to complete an photo workflow. Unfortunately an applescript will be needed for that, and I don't know anything about it. I hope someone of you already solved this ( I didn't find a solution in the forum).
I want Hazel to monitor a folder with images. I'm working with Lightroom and export the photos into a specific folder. Those images should be imported in a new album into the Photos app. The name of the album should match one piece of the folder structure.
My Folder structure :
Year/Month-Describtion/bearbeitet-jpg/IMG_0001.jpg
The files from folder /bearbeitet-jpg/ should be imported to Photos into an album with the name "Year/Month-Describtion".
Does anyone of you has an applescript for that?
I know there is an action "Import to Photos", but it cannot create those named albums.
