So I have a folder that has a red indicator in the menulet.

Image

- hazel has full disk access.
- it works for awhile then stops working
- this red indicator is no where else.

what am I missing?

What's the full path of that folder? It definitely does not have that in Finder? That indicates a permissions/access issue. Hazel does not add that icon; it is what the system hands to Hazel when it asks for the folder's icon.

Can you answer my other question about how it looks in Finder?

sorry... the finder icon doesn't look like that just looks like a regular folder. If I remove and add it again it goes away in the menulet interface but then returns.

Resolved: The issue was that the hazel helper needed the same access.
