Renaming using elements from file name

Page 3 of The Grinnell Herald,published in Grinnell, Iowa on Tuesday, May 17th, 1904.jpeg
The desired result would be:
IA-GrinnellHerald_1904-05-17_p-3.jpeg
I searched the forum and couldn't find this particular set of issues. I've been away from Hazel for several years, so I'm really struggling. I would prefer a rule or sequence of rules that would work on files clipped from different newspapers, without the need to create a duplicate set of rules for each unique newspaper title or location.
Any guidance would be appreciated.
