Move files into subfolders based on date

I have a folder structure that I use to maintain paperless office via Devonthink. The top folder is 'Months'. Certain scans from my scansnap go here. I then have Hazel change the file name to indicate the data added. What I would like to be able to do is have Hazel then move the file(s) to the appropriate subfolder. I have a subfolder for each month of the year (January, February, etc.). So far, I can only get this workflow done if I use a rule to color the files with a particular date, then have Hazel sort them into subfolders base on color. I'd like to eliminate user intervention once the scan has taken place if possible. Thanks.

Why not just use the "date added" attribute in your "Sort into Subfolders" action?

I can see where that would get files moved into a folder for the current month, but how would I set it up so that the files would always go into the correct folder throughout the year?

I'm not sure what you are asking here. If you put the date added in the sort pattern, it will create (or use, if it already exists) a folder with that date. You can modify the date pattern to only use parts of the date you want. Try playing with it to get a feel for how it works.
