I have a folder on Box.com I'd like to sync to iCloud and Dropbox. I've successfully set a rule for this but it only runs when I activate it manually. I've turned off some other Hazel rules for other folders and looked in the log but there's nothing there. When the rule should run there is literally nothing in the logs. When I manually run it the logs are there.

You'll need to check the dates on everything underneath that folder. The folder itself is only modified when files are directly added or removed from it.
