Hazel is not adding days in Custom Date/Adjust Date

Using Hazel 4 on a Mac running Sierra. Receive bank statements with the beginning date of the month followed by the ending date of the month. Hazel reads the 1st occurrence of the date but not the 2nd occurrence. So I set up a Contents contain match rule with a custom date to match the 1st date occurrence. I set up a Rename rule with the same custom date and tried to Adjust Date by adding 30 days. The Rename rule does not add the additional days to the date.

Can you provide a screenshot? Also, does the date change at all or it changes, but not by 30 days?

Lastly, have you tried specifying matching the 2nd occurrence so you can match the end date instead?
