Custom date field - trouble selecting the correct date

I've been using and loving the custom date field but have run into a wall getting Hazel to select the correct date on a bank statement. No matter what date I point Hazel to in the statement (1st from top, 2nd from top, etc.) the wrong date is being selected. Any ideas how to use this feature in this case so I can get the correct desired date in the file name? Thanks in advance-

Can you provide more details on the document in question and the date format? Also, in the preview, click on the badge for that condition. It will show you the document text which may reveal some interesting things.

The document is a bank statement and there are two dates at the top, the last statement date and this statement date. The dates are written in the document "September 15, 2016" and "September 30, 2016" (short time window due to new account!).

The badge (the green check mark) when selected indicates "Sep 15, 2016" after the custom date name. I have 2nd from beginning selected as the criteria in the setup.

Click the badge. That will show you the actual text in the order as seen by Hazel. It may not be in the order you think.
