I am receiving this OS message (above) every time I login to my machine.

It's a bug in OS X where it doesn't properly clear the quarantine flag. This usually happens when you install it machine-wide, where you need admin access.

The easy solution is to remove it (just throw away the pref pane file, don't click the Uninstall button) and install again, this time doing it only for your account.

Or you can use the commandline, if you are ok with using Terminal. Email support if you want these instructions.

Is this the solution you are advocating versus using Hazel's uninstall (see image):

Should I expect to have to reinstate any license information?

That's how I'd do it.

Nope. The ~/Library/Application Support/Hazel folder (which contains the license, rules, etc.) will be retained if Hazel's Preference Pane is removed using the method shown in your image.

I recommend keeping a permanent "database" of software licenses and personally use 1Password for that purpose.
