How to save and editable PDF and have it remain editable.

Talk, speculate, discuss, pontificate. As long as it pertains to Hazel.

Hi guys, and thanks for any time spent helping me before hand.
My problem is that I have 12 insurance forms that I need to fill out weekly so the insurance can reimburse me for my meds.
The forms are exactly the same, the only thing that varies are the dates and any price differential. All the insurance info, names, meds, etc., are the same every time, and it is very time consuming, and not to mention annoying, to fill and refill all the forms every week.
I have tried to used google chrome, and the trial version of adobe acrobat to try to fill out the forms and save them in a way that they are still editable, but I have come up short on how to do it.
Can some one help me out with this? Preferably in a way that is not too expensive.
Thank you in advance.

This problem seems outside of Hazel's functionality. Maybe you can do it with Shortcuts or Automator and you might get more answers on a forum dedicated to either of those.
